Required Medical Forms
All medications and completed medical forms must be brought to school by a parent/guardian by the first day of school. Medical forms are available for download below. To request a hard copy, please email firstname.lastname@example.org
or call 856-667-1013.
Any student whose parent / guardian has not submitted the completed, state-mandated forms prior to the first day of school WILL BE EXCLUDED from school until all requirements are met.
Requirements for New Early Childhood Students
All NEW 2-Year, 3-Year and Pre-K students must submit the Student Health Inventory (C) and Preschool Physical Examination and Immunization Record (F). In addition, all Early Childhood (2-Year, 3-Year and Pre-K) students need a flu shot as soon as possible, with documentation sent to school.
Requirements for All Kindergarten Students
All Kindergarten students must submit the Health History & Appraisal Form (B) and Universal Health Record (J).
Requirements for All NEW First through Eighth Graders
All new incoming students in grades 1 through 8 must submit the Health History & Appraisal Form (B) and Universal Health Record (J).
Requirements for All Sixth Graders
All sixth graders must provide documentation of Tdap and Meningococcal vaccinations prior to the first day of school. The New Jersey State Department of Health also recommends that children have a physical prior to entering sixth grade. Please have your child's doctor complete the Health History & Appraisal (B) form. If you child has an appointment with the doctor past the first day of school, a note from the doctor or an appointment card with the date of exam is required before the first day of school.
Required Sports Physicals
Completed Sports Physicals for students in sixth through eighth grade who will be participating in fall sports must be submitted as soon as possible. Students will not be permitted to practice or play sports until they have been cleared by a physician. The Sudden Cardiac Death and Concussion Policy verification form, signed by both the parent/guardian and the student athlete, must be submitted to the coach prior to practice or play.
